Direct Deposit
Add, change, or cancel your current setup; complete and submit to Finance.
Emergency Contact Form
In the event of an employee emergency, the information we currently have on file may/will be used; please complete this form if you wish to update your emergency contacts.
Designation of Person
In the event of an employee death, warrants and checks that would have been payable had the employee survived, will be paid to their designated beneficiary.
Changing this form also updates your State Tax
You should complete this form only if you claim a different marital status, number of regular allowances, or different dollar amount to be withheld than you claim for federal income tax withholding.

Pre-designation of Personal Physician
In the event you sustain an injury or illness related to your employment, you may be treated for such injury or illness by your personal medical doctor (M.D.), Doctor of Osteopathic Medicine (D.O.) or medical group.
DWC 1 Form
Were you referred for medical treatment? Complete the DWC1 and return it to Human Resources within one working day.

INJURY AND ILLNESS PREVENTION POLICY
The City of Pinole strives to maintain a safe and healthy work environment for all employees. We recognize the responsibilities for safety and health are shared. As your employer, the City is committed to providing leadership and support for the effective implementation of the City’s safety and health programs. The City Manager, department heads, managers, supervisors, and employees are required to comply with all applicable federal, state, and local safety laws and regulations. Each department is required to conduct its operations and activities in a safe manner to minimize the risk of injury to employees and the public. No employee will be required to conduct any task that is determined to be unsafe. The City Manager, department heads, managers, supervisors, and employees are expected to report potential safety hazards without fear of retaliation and with confidence that safe and healthful conditions and practices will prevail in the workplace. It is the policy of each City department to identify and minimize potential risks inherent in the operation of various programs, services, facilities, and equipment.
SB-553 WORKPLACE VIOLENCE PREVENTION PROGRAM
The City of Pinole (“City”) is committed to providing a work environment that is free of disruptive, threatening, or violent behavior involving any employee, appointed or elected official, volunteer, contractor, client, and/or visitor. Our policy is to establish, implement, and maintain an effective plan as required by SB 533. The regulation requires us to establish, implement, and maintain a workplace violence prevention plan for purposes of protecting employees and other personnel from aggressive and violent behavior at the workplace.
REPORT WORPLACE WRONGDOING
The City of Pinole (“City”) strives to provide a professional work environment where all employees can work together comfortably and productively, free from harassment and discrimination. The City prohibits harassment and discrimination based on an individual’s race, religion, color, creed, national origin, gender, sexual orientation, marital status, age, or physical or mental disability, regardless of whether that harassment is targeted specifically to an employee or independent contractor; the City is also committed to providing a workplace free of sexual harassment.
Wrongdoing can come in the form of harassment, discrimination, theft, violence, fraud, unsafe acts and other forms. You can report wrongdoing in a variety of ways:

– the Employee Reporting Line (877) 651-3924;
– or you can go online at www.employeeprotectionline.com.
The Employee Reporting Line is monitored 24 hours a day by an independent third party. You can make your report without disclosing your identity, and calls and online reports will not be traced.
